Now that I have kind of put an end to the sales aspect of my existence and am only concentrating on more esoteric research, I am looking at my past performance with a critical eye and wondering if the entire aspect of "health management" is not too skewed by commercial imperatives to be successful. Let me explain my thoughts...

Germany has the biggest market in the world for blood pressure monitors. Last year alone, about 1,8 million blood pressure monitors were sold in Germany. From the point of view of a manufacturer of blood pressure monitors, this is wonderful. It is good business. However, we must also ask a question that has an unpleasant answer... Is this also good health management? Unfortunately, from a person who prefers to view blood pressure monitors as useful only if used in the context of a managed healthcare program, the answer is an emphatic NO! Do German people who buy a blood pressure monitor have a better cardiovascular risk profile than those who don't? The answer is "not really". The reason for this is that many people who buy a blood pressure monitor do so to assuage their guilt about their poor lifestyle but do not really change their lifestyle.

In the healthcare industry, "management" is a magic word that blankets all manner of excuses and that often fails to address basic issues such as "disease education", which is the axis on which healthcare should be balanced. There are as many definitions of disease management as there are disease management programs. The confusion is perhaps rooted in a fundamental paradox: In disease management, we really should try and manage patients, not diseases.

It is absolutely pointless to tell a patient to purchase a blood pressure monitor and to monitor himself regularly if the patient is not made aware of what he is attempting to control and what his parameters are. Although I subscribe to the adage of "if you cannot measure it, you cannot manage it", I also believe that an understanding of the parameters of hypertension is essential to its eventual successful control.

Good communication between the physician and the patient lies at the core of the successful management of hypertension. Good information about blood pressure and high blood pressure, about risks and prognosis, about the expected benefits of treatment and about the risks and side effects of treatment is essential for satisfactory life long control of hypertension.

All right, we know all this. It is a given. So the Microlife BPWatch is great technically, is also great from a managed healthcare point of view, it is a fantastic device that follows the recommendations of the ESH and of the AHA (American Health Association).

Question is, will it sell?

This all depends on whether the sales and marketing team for this product are able to rise to the occasion. It will be a little more complicated to sell the WatchBP than to sell a standard home-monitoring device because whereas the home-monitoring market is principally price-driven, the WatchBP is scientifically driven. This might prove to be a problem long-term for the WatchBP as it does not yet have the distribution networks the product deserves.

This rambling discourse might be an occasion to discuss tomorrow the concept of HCD (Human Centered Design) where ALL aspects of a product are determined on paper before the first prototype is made. When I say ALL, I mean not only the technical or scientific aspects, but also the sales and marketing aspect of a new product, so that the final product is an all-encompassing experience that automatically positions itself at the right level in a market and so that the final product is not only beautiful and intuitive to use, but is also a delight for the end user.
